Policy & Parent Permission Form

  1. Student must be either 15 years, 9 months of age by the start date or 16 years of age by the last regularly scheduled class and is required to provide a copy of his or her birth certificate with their application as proof.
  2. Student must provide documented proof that they have passed an eye test; 20/40 vision (with corrective lenses if necessary) is the State requirement.
  3. Student agrees to attend all scheduled classes. Students are not allowed to miss classroom work unless requested by parent due to illness. A student’s excessive absence will result in an incomplete or failing grade for the course and will be asked to leave the class with no refund.
  4. Student must attend & participate in 30 hours of classroom activity. If a student is absent from class then he or she must seek makeup work from their class instructor in order to receive credit toward the classroom hour requirement.
  5. Students arriving more than 5 minutes late to class will be considered absent.
  6. Students are not allowed to enter any other area of the Londonderry High School before, during or after class. Failure to abide by this rule could result in class dismissal with no refund.
  7. Students must bring their textbook, driver’s manual and a pen to each class.
  8. As part of the classroom activities each student must:
    1. Pass 4 quizzes (grade 80%) derived from the State Driver’s Manual
    2. Satisfactorily achieve a passing grade (grade 80%) with regard to homework assignments
    3. Achieve a passing grade (grade 70%) with regard to the Final Exam. Student will be given the opportunity to retake the final if necessary.
  9. Cancellation of class due to bad weather: driver education classes & driving lessons are cancelled on days which LHS cancels due to bad weather unless you hear otherwise from your instructor.
  10. Student must satisfactorily complete 10 hours of driving with an instructor and 6 hours (minimum) of observation of another student’s driving also with an instructor.
  11. Students must demonstrate their ability to perform the following to their instructor
    1. Track straight down the road
    2. Recognize safe gaps in traffic
    3. Recognize and obey stop & yield signs
    4. Drive within the posted speed limit
    5. Comply with Right of Way Rules
    6. Back straight within a foot of the curb
    7. Park in parking lots and parallel parking spots


  1. Students are required to document 40 hours of “out of class” driving to the Department of Motor Vehicles when testing for their license. The State Form will be provided by the Driving School at the beginning of the course.
  2. Driving lessons are scheduled in advance. If a student misses a driving lesson without notification, then he or she will be required to pay a missed driving lesson fee of $50.00. Settlement of the fee is a requirement of the course.
  3. Cancellations of a driving lesson must be done 24 hours prior to the scheduled lesson, please call your driving instructor directly to avoid the penalty fee.
  4. No refund will be given after the class start date.
  5. Disruptive behavior, possession of alcohol or any illegal substance (internal or external) will not be tolerated during any portion of the driver education program. Dismissal from the class with no eligibility for a refund will result as violation of this policy.
  6. Successful completion of the course requirements:
    1. Driving & observation hours satisfactorily completed (sec. 11)
    2. Homework, quizzes & tests satisfactorily completed and passed (sec. 9)
    3. Textbook returned in the condition it was given
    4. Account paid in full (includes cancellation or textbook replacement fees if applicable)
  7. If homework, quizzes or final are not completed within 1 week of course completion, the student automatically fails the course with no refund. Student will be required to register for a future available course offering at the then current tuition rate.

I give permission for ________________________________ to enroll in the Driver Education course as offered by Mr. H’s Driving School. To the best of my knowledge, he/she does not have any physical, mental or other disability which would interfere with the safe operation of a vehicle.

Per recommendation of the Department of Safety, please answer the following questions.

  1. Are the driving privileges for the person enrolling in the driver education program currently under suspension or revocation?                  Yes      No
  2. Is there any pending action against the person enrolling in the driver education program which could cause the driving privileges to be suspended or revoked in the future?                                                                                            Yes      No


If the enrollee’s driving privileges become suspended or revoked during the program, any such action will immediately be reported to the State’s driver education program administrator.
